Abstract
Background and Objective: Facial aesthetics and self-presentation assume significant importance in modern society and augmenting interest in orthodontic treatment practices. Fixed labial, or regular, appliances, namely braces, and clear aligners are the most common orthodontic modalities, and these two vary in their effects on perioral soft tissues and speech. The objective of this study was to compare the immediate effects of fixed labial appliances and clear aligners on perioral soft tissues and speech.
Methods: Anin-vivo comparative study was conducted at Vishnu Dental College. The samples included an equal number of orthodontic patients who were randomly allocated into two groups: fixed labial appliances (n = 20) or clear aligners. Data were analyzed for the immediate effects of the appliance on baseline measurements (pre-treatment) and after 1 week of appliance placement. The perioral soft tissue changes were assessed using standardized pictures taken outside the mouth, while the effects of the appliance on speech were assessed using a visual analogue scale (VAS). Comparisons within and between groups were made using paired and independent t-tests.
Results: S-line and width increases were significantly larger with fixed appliance use when compared to aligner appliance use (p < 0.001). Except for LAFH, S-line and width, there were also LAFH, S-line, and width parameter changes of statistical significance across all parameters for the cases treated with aligners (p < 0.001). Fixed appliances and aligners had a statistically different S-line (p < 0.001), whereas LAFH and width intergroup differences were not statistically different. There was no statistically significant difference between the speech assessment results of the fixed appliance/hygiene and aligner cases (p > 0.05).
Conclusions: Aligners tend to influence vertical dimensions, whereas fixed appliances predominantly affect sagittal parameters. Both treatment modalities demonstrated comparable effects on speech in the immediate phase following appliance placement. Understanding these differences may help clinicians tailor orthodontic treatment according to individual patient needs.
